This proposal removes the `:!` syntax for generics and templates in
favor of keywords (`generic`, `template`, `runtime`) and contextual
defaults for phase. It also replaces `:?` with `fwd` and introduces
`exttype` for extended types.

Incorporates changes from these proposals:
- #2022
- #2875
- #3262
- #3763
- #3848
- #5434
A small amount of updating was done to lambdas.md and variadics.md to
harmonize with these changes.

We've talked about adding the title to the filename several times over
the years and it seems really valuable. This requires us to compute a
"slug" for the title spelling that can be part of the filename.
Beyond that, we crossed 7000 recently, and so it seems likely that we
will need to add digits sooner rather than later here, so this goes
ahead and moves us to 6 digits so we don't have to adjust again for a
reasonable length of time.
To implement this and ensure we can sustain it going forward this adds a
tool to our pre-commit that validates (and corrects if needed) the
filename.
In order to update everything and keep links working, there are a _lot_
of changes, but the most interesting for direct review are in
`proposals/scripts`.

Update the syntax for class (and interface/`impl`) methods to move
`self` into
the parameter parentheses `()` and make the type in its binding optional
(defaulting to `Self`). Introduce the `static` keyword for non-instance
member
variables to indicate static storage. Reflects the decision in leads
issue
[#6931](https://github.com/carbon-language/carbon-lang/issues/6931).
Updates the directly relevant design, but leaves a systematic update of
examples
to a future PR.

This proposal introduces the concept of a _form_, which is a
generalization of
"type" that encompasses all of the information about an expression
that's
visible to the type system, including type and expression category.
Forms can be
composed into _tuple forms_ and _struct forms_, which lets us track the
categories of individual tuple and struct literal elements.
The proposal PR also adds `ref` bindings to the pattern matching
documentation,
but that is not part of the proposal itself; it's just bringing the
documentation
up to date with proposal
[#5434](https://github.com/carbon-language/carbon-lang/pull/5434).

Require language design proposals to either update the design documents
to
reflect the proposed changes, or add "TODO" comments to mark where those
changes
will be needed, with links back to the proposal. This is intended to
ensure that
the design documentation accurately informs readers about the current
language
design, without excessively burdening the proposal process.

Also tries to use more precise language when talking about:
- implementations, to avoid confusing `impl` declaration and definitions
with the `impls` operator used in `where` clauses, an issue brought up
in #2495 and #2483;
- "binding patterns", like `x: i32`, and "bindings" like `x`.

I was doing this for #851 initially, but I think #438 and #826 hadn't made it in (possibly intentionally due to #851? I don't recall).

Propose the decision from #542, noting implementation from #563
Also integrates some of #339 into `variables.md` because that's actually how this started, looking for a proposal reference for #542

Adopts new copyright and markdown toc checks.
The new toc check generates the toc header, so that's why all the md files changed (this had felt better to me for the long-term, more auto-generated content)

This proposal should be considered a starting point of the language design. It's not intended to be final; language details may change. This is intended to offer a reasonable starting point for:
- Example code.
- Conceptualizing Carbon at a high level.
- Reasonable, but not necessarily final, approaches to features in README.md.
- If any idea is obviously bad, we can clean it up here.
This proposal is not intended to achieve:
- A whole language design.
- This is way too much work for a single proposal; this is a skeletal framework only.
- As we work on feature-specific designs, we may decide to use other approaches. That's fine: we only need somewhere to start.
- The summaries in README.md may be expected to change over time.
- Feature-specific files aren't intended to be well-written or comprehensive. They are a quick jot of prior thoughts.
- We want to avoid getting stuck on language details that we should consider
more carefully regardless. If you're passionate about a feature, please feel
free to start a new proposal for it.
- Each and every aspect of the suggested overview should be subject to careful
examination and justification before it becomes a settled plan of record.
Chandler started this with https://github.com/carbon-language/carbon-lang/pull/22. I've taken it over with the following changes:
- More of a directory hierarchy.
- Trying to thin out the main file (now README.md) to lighter summaries of features.
- Details/rationale/alternatives should be in feature-specific files.
- Draft files are linked as references where added.
For an example of how we may proceed with feature-specific designs, see https://github.com/carbon-language/carbon-lang/pull/80. In this structure:
- docs/design/README.md mentions interoperability, with a light overview.
- The light overview is not yet in https://github.com/carbon-language/carbon-lang/pull/80.
- docs/design/interoperability/README.md goes into more depth on interoperability, covering key points of the approach.
- Individual files in docs/design/interoperability/* go into more depth on interoperability.
Simple designs may not have a subdirectory. All current feature-specific designs do not -- they may be moved later.
